Wireframes were created for the patient, physicial and caregiver dashboard

Affinity diagraming was used to define opportunities and the functional requirements for the application. A network diagram was created to visualize interaction architecture of the application

Problem:  People with congestive heart failure are often readmitted to the hospital

Scenarios were created to depict the current situation and visionary situation with the presence of the product

System architecture was created based on funtional requirements
